Dr Shen-Wen Cheng

MBBS, FRACGP

Dr Cheng was born in Taichung, Taiwan, in 1980 and came to Australia 2000. He completed his medical degree at the University of Adelaide in 2007. His internship was undertaken at the RAH and his residency at the QEH. During his resident medical officer year he completed a research paper on using CT technology in predicting stroke and presented this at the Royal Australian & NZ College of Radiologists annual scientific meeting in 2009. He has also done training in radiology at the Royal Hobart Hospital for a couple of years.
Dr Cheng has been practicing General Practice since 2012. Since then he has trained in a number of good quality clinics including Adelaide Health Care, Medical HQ and Ladywood Clinic. He also spent 6 months as a GP Registrar at Adelaide Unicare’s two practices in Adelaide’s CBD.

Dr Sanjeev Joshi

Dr Mahesh Chhanabhai

Dr Huma Aziz

Dr Huma Aziz has returned to Hampstead Health after completion of her education & training in General Practice. She is now Fellow of Royal Australian College of General Practitioners (FRACGP). Dr Roopali Verma

How To make an Appointment

To make an appointment, you can use our online booking system on this page.  Appointments can also be made by calling reception on 8162 9817.

All consultations and procedures are on an appointment basis. Emergency cases – whether illness or accident will be seen as promptly as possible. Reception staff must be notified or urgency. Appointments are made at 10-15 minute intervals. Longer consultation or procedure times are available if required. Please notify the receptionist when making your appointment. If more than one family member is to be seen by the doctor, please advise the receptionist at time of booking so an additional appointment can be booked. Emergency or longer consultations could delay your appointment. For continuity of care, patients are encouraged to make appointments with the usual GP whenever available.

Please fill in this New Patient Form and bring it along to your first appointment. Alternatively, you may fill one in prior to your appointment.

HOME VISITS

We ask patients to make every effort to attend the surgery for consultations, however we will visit our regular patients at home wherever possible if their condition prevents them from coming to the surgery and if they live within a reasonable distance to the surgery.

What do I need to do?

When you receive an SMS, open the message and click on the secure link. You’ll be taken to a screen where you will need to verify your personal information, including your last name and date of birth. Once you have verified your identity you can read your recall health message. From here you can book your appointment for a time convenient to you.

Hampstead Health is a family focused general practice which enables equal access to medical services by bulk billing all patients with a valid Medicare Card.

Our philosophy is simple – to provide the highest level of general practice care to all families and members of the community. Our committed and friendly team of doctors, nurses and administrative staff share a common passion in striving to provide the highest level of comprehensive health services to all members of the community.

While Doctors and Nurses treat emergency situations that may sometimes occur during a general appointment, the practice is NOT an emergency clinic. Hampstead Health does not have all of the necessary life saving medical equipment that hospital emergency rooms have.

If you or someone you know is in a medical emergency, or you are suspecting the situation will become an emergency, you must call TRIPLE ZERO (000) immediately.

Hampstead Health is supported by a committed team of nurses and administrative staff.

Nurses are available each day and evening for procedures such as immunisations, chronic disease management, pap smears, wound care, B.P checks, health assessments and more.

Our Practice Manager is Kevin Gray.

Changes to our billing policy

From 1st July 2024 there will be a $35 gap fee for face to face and telehealth consultations for patients with a Medicare card

Patients who hold a current Medicare card and are15 years old or younger, healthcare, pension and DVA GOLD / White (eligible conditions) card holders, will continue to be bulk billed at all times.

To be eligible for a Telehealth Medicare Rebate, current legislation requires that you have had a face to face appointment at the practice in the last 12 months. If you have not visited our practice in the last 12 months, you do not qualify for the Medicare rebate. This Telehealth legislation affects all medical practices.

Please note, payment for the gap fee as well as the Medicare item fee is payable up front at the time of consultation Payments are accepted by credit card, debit card or cash (surcharges apply for card payments)

Patients without a Medicare card will be charged the full fee at the time of the consultation.

*Telephone consultations will also incur a Stripe online payment surcharge
